His-purkinje dysfunction
WebbThe sinoatrial node may become dysfunctional and fail to depolarize. Failure to depolarize may be intermittent, or persistent for longer periods or even permanent. This could potentially result in cardiac arrest but it rarely does, because the absence of sinoatrial impulses will allow for one of the other pacemakers to establish a heart rhythm. WebbThe AV system consists of the atrioventricular node (AV node) and the His-Purkinje system. These structures conduct the atrial impulse to the ventricles. Impulse conduction through the atrioventricular node is slow. This is explained by the scarcity of gap junctions in the cells of the atrioventricular node.

WebbConclusions—Permanent DHBP is feasible in select patients who have chronic atrial fibrillation and dilated cardiomy- opathy. Long-term, DHBP results in a reduction of left ventricular dimensions and improved cardiac function. (Circulation. 2000;101:869-877.) Key Words: bundle of His n pacing n cardiomyopathy
WebbHowever, surprisingly and for reasons still unclear, most patients with pacing burdens close to 100% do not develop LV dysfunction or remodeling . “Conventional” RV pacing (apical) causes myocardial activation almost in reverse to that which occurs in intrinsic physiological antegrade conduction using the His-Purkinje system.
